Please read over, fill out, and return the
forms included in this packet by August 6, 2010.
A brief description of each form is included below. You may bring
them to practice on June 16 or the week of August 2-6. You can also
mail to WHS 2121 Bethel Rd, Weatherford, TX 76087.
1)
Parent Permission for
School Sponsored Trips: (Bus Form)
Release to ride transportation provided by the
school district to and from events.
2)
WISD Medical Release/Emergency
Information:
This form is used if an emergency arises and I
need pertinent information. Parents will be notified in case of an
emergency!
3)
Internet Publishing
Releases (Media, Internet, and Video):
These forms give your permission to publish
your dancer’s picture to the paper, website and video.
4)
The Blue Belle Constitution
and Handbook:
This is a detailed account of the goals,
requirements and responsibilities of the Belles.
Please sign the back page (both parent
and Belle) to show that you have read over the handbook.
This will help the year run smoothly if everyone knows what to
expect. Thanks!

Blue Belle Profile:
This is information gathered for each Belle.
6)
Blue Belle Parent Profile:
This is information regarding Belle Parents
which will be used for notifications and to get to know you better.
7) Blue Belle Parent Volunteer
Form for WISD:
All parent volunteers must have one of these
forms on file to work with students of the WISD.
8)
UIL Physical Form:
All Belles must have completed physical by
August 6th 2010. Buy your coupon to Robbins Peditatrics
